The missing muzzle flashes and 'ghost deaths' when it looks like he's not shooting are a frequent bug of kit swapping. Switch kits and quickly start firing. Sometimes the weapon won't play the shooting animation, the muzzle flashes, or will even appear to have 0 ammo and 0 clips, but it will actually be shooting and you will be able to see the hit 'X'. Happens all the time if you kit swap. If you haven't seen it happen, likely just means that you're the type of person who tries to reload and dies instead of stealing someone else's kit

well the muzzle flash is already explained. the thing with the nades, is if you noticed you see 4 explosions. i cut the clip short of the first nade i threw. so the first one blew in the beginning of the clip then the 2nd, 3rd and 4th. just the first wasnt shown being thrown.
